The Nigerian Safety Investigation Bureau (NSIB) has commenced an investigation into the helicopter crash in Igbokoda, Ondo State, as efforts continue to establish the circumstances surrounding the incident.
According to the NSIB, three bodies were initially recovered during the search and rescue operation, while its preliminary investigation later confirmed the recovery of five passenger bodies.
The investigation has also identified several safety concerns, including the absence of a flight data recorder on the helicopter, contrary to applicable Nigerian regulations, as well as the use of non-standard radio phraseology and a deferred radio altimeter defect.
The incident has renewed concerns over the safety of helicopter operations, particularly offshore flights and operations conducted in challenging environments.
Investigation to Determine Cause of Crash
The circumstances leading to Monday’s helicopter crash in Igbokoda remain unclear, with aviation investigators expected to examine several factors as part of the ongoing probe.
These are expected to include the wreckage, flight records, prevailing weather conditions, aircraft maintenance history, communications between the crew and air traffic control, as well as other available evidence that could shed light on events leading up to the crash.
The NSIB has cautioned against speculating on the cause of the accident until the investigation is concluded and official findings are released.
The bureau has consistently maintained that its mandate is focused on fact-finding rather than fault-finding, with the objective of determining the circumstances and probable causes of accidents and making safety recommendations to prevent similar occurrences.
Meanwhile, the immediate concern in Igbokoda remains the fate of those aboard the helicopter, as emergency responders continue their efforts at the crash site.
Families and residents are awaiting further official information on the total number of people involved and confirmation of any survivors.
The NSIB and other relevant authorities are expected to provide additional details as search and rescue operations continue and the preliminary assessment of the crash site progresses.
